This is a 38 year old mother of two who presented with asymmetric breasts with the right being smaller than the left. Discussions with patients in this category need to understand the other aspects of the breasts that may be different. This includes in this patient the asymmetry of the nipple areolar complex (NAC) and the difference in height of the Inframammary Crease (IMC).

When determining the approach to this patient, the various differences are considered. The right IMC needs to be dropped and the implants used need to be different by the amount that they differ preoperatively. It was determined that this patient's breast volume was about 50 cc's and therefore after review of many patient pictures, the implants are chosen accordingly. The patient wished to have silicone in a moderate plus profile. A 425 cc implant was chosen for the left and a 475 cc was chosen for the right. The insertion site was at the crease and therefore the breast unit, per se, was not entered as the implants were placed under the muscle. I feel that all implants (a few exceptions) should be placed under the muscle in that this allows another layer of coverage over the implant.

The patient in shown in the post op pictures at 3 weeks...yes 3 weeks ! The naturalness will continue to improve over about 6 months. Notice that the right nipple complex is still lower than the left. The pre op photos show them asymmetric as mentioned and if the patient wanted, the right complex can be moved upwardly at the time of the augmentation. This patient decided not to move the NAC and as you can imagine, she has her confidence restored. (Mommy makeover !)
