Transgender Albany: This young man struggled with his genetic sex for many years as do most transgender patients. They are misunderstood and feel isolated but as the groundbreaking decisions and openness of various lifestyles are becoming more understood and accepted, the patients that want to be transformed can have an easier time opening up for the corrections needed to get them to their goals. This patient has been on hormone therapy and you can see that his breasts have changed dramatically with the hormone treatment and the larger breasts that he had as a female are now ready to be removed to give him the male chest that he desires.

If a genetic woman has small breasts, then there is little need to do the mastectomies such as seen in this case. The scars can vary widely but I find that the best ones are made at the crease of the breast. The tissue is then elevated with the breast tissue off of the muscle and then "redraped" so as to allow the removal of all of the breast tissue. This leaves the chest completely flat. The nipples that were removed and set aside can then be brought back to the predetermined position on the chest wall and dressings applied. The "take" rate of these free grafts is well over 95% and in fact, in transgender patients, I have never really had any part of the nipples not make it.

The scars are explained quite easily for the patients as noone other than perhaps another transgender patient will have a clue as to the reason for them. Quite frankly, transgender patients have already accepted totally that the scars are a small price to pay for the shape of their chest after the surgery.

This case shows the most difficult of the transgender breast transformation -  going from a female to male. It requires an incision that will remove the breast tissue AND take away the large amount of extra skin that is left. Patients in this category usually know this type of approach as it is a free nipple breast reduction but essentially removing all of the breast tissue.

For those with a high nipple position, you can use the same approach through an areolar incision as is often used for a gynecomastia reduction.
