Lovely 36 y.o., status post 2 pregnancies and breast feeding/pumping for over 6 months. She has the beginning of ptosis  ( nipple/areola complex drifting below the lower breast crease.) with significant loss of breast volume, with a bra size of about 34B. She is 5’7’’ and weighs ~ 130 lbs.

When ptosis occurs, just placing a prosthesis where the normal breast mound was located will only accentuate the downward migration of the nipple. Therefore, a relocation of the nipple to a more superior (original  anatomical location) is required. Fortunately this patient was just at the cusp of having to do this. We discussed her options of lowering the breast crease, internally lifting the nipple areola complex and choosing a prosthesis size that would compliment these surgical maneuvers. After comparing multiple sizes, we arrived at 425cc. moderate plus profile silicone prosthesis. The creases were lowered, the breast tissue was internally lifted, and the implants were places below  the chest wall muscles to give a more natural slope from chest wall to breast mound. Placing the prosthesis in this location also decreases the potential for excessive scarring to occur; another reason why the consultation is so important. Together, the patient and I developed a plan to attain her desires.
