Breast revision/augmentation/lift/asymmetry/capsules

This 54 year old woman had an augmentation mammoplasty by another plastic surgeon.  She has developed significant asymmetry with a contracting capsule on the left with an implant shift.  Her right breast is more droopy and there is considerable asymmetry of the nipples in both shape and positon in relation to each other.

  When seeing a case like this (and I find some of the severe revisions quite interesting), a plan needs to be figured out such that all of the issues are identified and then how to correct the irregularities.

  During the first augmentation, the irregular nipple complexes were not identified or if they were, nothing was done about their asymmetry.  Also, there is more surface area of the right breast and without reducing that area to match the left breast, then there CANNOT be a match.  The capsule is forming a ball-like appearance on the left. 

  The plan is to approach both breasts at one procedure and to do an asymmetric bilateral donut mastopexy.  This allows us to match the nipple complexes  in that they are very irregular in her pre op pictures (post op result from the other surgeon/surgery).  The capsules need to be modified on both sides and taken out where necessary (capsule = a normal body response to any implant.  If it distorts the breast as it is doing on the left, then the approach needs to lessen its effect and soften the denting above the left breast caused by the severe contraction of the scar/capsule).  The patient had 350 cc saline implants filled to 400 cc.  Notice that the implant volumes are the same.  This is important.  The "match" of the breasts is done first at the table and this allows the same volume of the breasts to be over the muscle with the same skin area.  Then the patient has the implants placed in the submuscular position and they are of equal size.  With this approach, the breasts will act in the same manner as the years and decades pass.

  The patient is seen at 4 months post op.  As you can see, the nipple complexes are much improved but still pink but  this will settle down over 6 months to one year.  The size and shape are much better and essentially the patient and the doctor are quite happy.

I am presenting this case because I do a great deal of revisional breast surgery and it is the "plan" that gets the win.  The patient tells me what bothers her and I tell the patient what concerns me.  We come to a mutual decision as to the method to achieve the best outcome as the options are decided.   It just makes sense to work with each others ideas.
