This is a 33 year old lady who lost over 130 pounds through bypass surgery. She has a vertical scar in the midline where the surgeons gained access for the bypass. Notice too that the patient has a transverse scar on her right upper abdomen. This was a previous scar from a gallbladder surgery. With a case like this, it is necessary to see where the goals are for the patient. Obviously, she wants to be able to get rid of as much skin and fat as possible. She also could use an asymmetric breast lift but she decided to only do the abdominal surgery. If this patient has ONLY the transverse incision approach to the tummy tuck (Please see the liposuction and tummy tuck category to see many different approaches to body contouring), then she will not have the tighter tissue. There will be too much tissue on the abdomen left. For this reason, I chose to use the midline scar that she has and tightened her from the horizontal and vertical direction. This allowed so much tissue to come out that the scar mentioned for the gallbladder is now...OUT..with the rest of the excess abdominal tissue. This patient is very new so the scars are red but they will turn a more natural color with time. Very happy patient. Very happy doctor.
