This is a 28 year old mother who desires her breasts to look more youthful because of the damage to the tissue from pregnancy and breast feeding. Note that there is a loss of tissue in the breasts and the drooping that will prevent her from having any possibility of "perkier" breasts without something being done for her.When she presented to the office, I told her that she would need a lift and for the best results, a "filler implant". I use this term in that the total purpose for the implant is for the fullness above the nipple complex and if the patient desires, she can go even more fuller. The lift without the implants would give her a better nipple position but the sloping above the braline would exist and take away from the more youthful look of the fullness in this area.We decided to do the lift and implants. The implants are under the muscle and therefore are really NOT part of her breasts but merely a "pusher" for the tucked breasts. Notice the fullness and youthfullness above the braline and also the decreased size of the areola complex that is much to big for tucked breas This is done in the majority of lifts. It is really part of the procedure.Now note the patient in the 4 month post op period. The fullness is there and the patient has one heck of a great time shopping. Imagine the monkeying (I just made that up) around that she has to do in the pre op condition. Now compare the confidence in the post op picture. Victoria Secret is not going to be passed anymore or she will not be asking for the gels and the push ups or fillers. She has her young age again in the breast area.The scars are around the areola and a vertical scar with a scar at the crease of the breast. Patients have to accept in a case like this that they will have a vertical component to the breast scar. Interestingly, the vertical scar is the first scar to become more normal color as it is on the thinner breast tissue. This is also a fact for the scar around the areola. Since she is only 4 months post op, the scars are in the transitory phase and will eventually blend to the normal color although, they are permanent. It is a tradeoff and each woman needs to decide if she is willing to accept scars for the improved results..The size of the resultant breasts is really determined by looking at about 50 cases in the consultation and then the patient can make an accurate determination of the size and shape that she desires.This is a very happy patient and this of course makes for a happy doctor too. Along this line, if something is just not right, then we fix it. If it can be done in the office, easy deal. There are sometimes small areas that we want to "tweek" and we do so as needed. Quite frankly, there are not too many times that any touch ups are needed.
