This 36 year old patient underwent breast augmentation with 420 cc saline  implants; submuscular position; inframammary incision. Shown four months post-op.  There are several things to note with this case that have to be considered during the consult discussions.  The presence of a moderate amount of breast tubing is seen on both breasts and the left nipple height and shape is different.  The creases are high and will need to be lowered to have the breasts accept the implants.  You can imagine the confidence that this lady has now with essentially a one hour surgery done as an outpatient.

  It is important to have a thorough discussion in choosing the implant size.  I show about 30 pictures during the interview and find the pictures that the patients like.  I then do the math to add or subtract to the implant size that the patient in the pictures being viewed has been given.  For example:  If a patient pre op in the pictures seen starts with a volume of 100 cc but the patient in the consult starts with a volume of 200 cc's, then the implant size used for the patient in the pictures must have a decreased size by 100 cc's.  Seems difficult but quite easy when you do thousands of these cases.  A patient will always want to discuss bra cup size but the pictures speak of the look.  Many patients tell me to give them implants that I think would be appropriate.  NO WAY !!!!   I need to see what the PATIENT thinks of size according to the pictures.  Then I know what will make her happy. 

  Bottom line:  Come in and discuss your case.  View the pictures.  Feel the implants.  See your implant size.  I place over 98% of the implants under the muscle.  So many arguments for this approach.  Many different insertion sites such as the axilla, the crease incision and an incision around the areola.  The advantages and disadvantages of each are discussed during the consult.

  So there it is in a nutshell.  Keep an open mind when coming in for the interview.  You might want to bring in photos of breasts that you like but I show so many cases that this is not really needed.
