This very shapely 28 year-old lady had difficulty in shopping for the correct bra and clothes. She did not like the difference between the breasts and felt that she was too small breasted. During the interview, there are several points that need to be made in a "Say it as it is" conversation. Patients do not look for sugar coating of their presentation. They have taken their time and anxiety to come in for a consult and want to have constructive criticism. I am very blunt with patients as far as what would get them the most ideal result. Note in this case that there is asymmetry . . . BUT . . . there are other important factors to consider. The decision for the size is critical and much time is spent looking at 10 or more implants . . . and deciding how to asymmetrically augment this patient. Once that is established, we look for other details to add to the confidence level. Note that the creases under this patient's breasts are very different. It would not be fair to augment this patient without changing them. They need to be more matched. This is done by moving them to a more ideal position and therefore accepting the implants in the most ideal way. In the post-op views, note that the creases are improved and now look very natural. This patient wanted to be full breasted. It should be noted that just about any size implant can be used and the desire for size . . . is determined by the patient and the doctor discussing what the patient feels is the best size. It is amazing the difference in the views of one patient as compared to another . . . and it is VERY patient specific as to the choice. It is NOT true that just a smaller implant will fit. This patient has a 475 cc saline implant filled to 525 cc's...and lesser on the larger size. However, this patient could have a 200 cc implant or an 800 cc implant . . . (yes . . . her tissue will take either and stretch accordingly . . . not necessarily the recommendation but size is in the discussion). She did not want the largest or the smallest. When going over the implant sizes...we do not shop for a bra size...we go for the ideal look...and then find the implant size that would be added to the patient's breast size...to get the final result. This patient is shown 2 months post-op . . . and she is very happy. Shopping fun . . . and a happy doctor too. Age 28525 cc total on the right . . . and 425 cc on the left.
