56 year old man with rather severe overgrowth of the glands of the nose (Rhinophyma) with the cobblestone effect seen on the surface. Patients in this category live quite pressured lives as they feel that nothing can be done for their condition and as you can imagine, they take quite a few glares and stares with visual and non visual interpretations of the cause of the condition. Most people think that if this condition exists, then the patient must be an alcoholic or something else that "caused" this situation.In fact, rhinophyma can be from many causes and it is essentially an overgrowth of the glands of the nose. They can go completely unhindered and as seen in this case, the irregularities of the surface and the crowding of the nostrils can be the cause of patients being reclusive and distant from the normal activities of life. This is not in every case but in most. You may see patients like this in the store or elsewhere but it is the patients that are staying out of the public that are in the larger category.The solution for this problem is a contouring of the nose with very small baby steps in each shaving/cauterization/dermabrasion/judgement aspects of the case. This is not a case for the inexperienced. This patients nose is "IN THERE" and you essentially have to try to "FIND IT". It is quite remarkable at the table as each area is improved and judged as the surgeon moves over the nose to try to get the best result.This patient is seen at 3 weeks post op. That is right, three weeks !!! After the shaping, it is noted that the nostrils are asymmetric but this would seem that this was the case before the contouring as you can see from the pre op nostril view. It is important to imagine this as you are contouring so you can stay safe in regions that can be improved only to the general pre op condition.You can imagine his self-esteem skyrocketing and his new found confidence in public. He needs many months to see the color come back to more normal and for us to see the final nose after the swelling comes down. I would say that if you asked this patient if he needs more time to judge, he would say no. He is a happy man.What a great postion to be in to instill confidence in a patient with a VERY difficult presentation. Do I really need to say CONFIDENCE IS BEAUTIFUL.Happy patient=Happy Doctor
