This patient is a very attractive shapely 34 y.o. woman who has had 2 children and breast fed both.    She did not like the toll that the pregnancies took and wanted to have some changes in her body contouring to restore her body to the pre pregnancy shape.

Our consult outlined the areas of concern.  She has an asymmetry and loss of volume in her breasts and she did not like the matronly droopy breasts, so common to Mom's.  She has a difference in the nipple position on the breast mound and this would need to be corrected at the same time as the breast lift with augmentation.

Another very important area for the patient was her abdomen.  She did not like the excess abdominal fatty tissue accompanied by the muscle loosening nor did she like the loss of shape of her waist/hips etc.  With the pregnancies the muscles are separated (rectus diastasis) and they will NOT grow back to the middle even with extensive exercise.  However, during the tummy tuck procedure, the muscles are tightened and I am quite agressive with the tightening.  This will bring the edges of the muscle together and even with more pregnancies, they will not separate again.  However, if one gets pregnant after the tummy tuck, there can be some stretching of the muscles in the vertical dimension but this can be improved with abdominal exercises. The tissue that was removed as the hanging over tissue will NOT come back even with weight gain (within limits of course).

 Our consult essentially goes like this.  We determine the areas of concern and then review the options to achieve the goals set forth.  For the breasts,  we review many pictures and decide what size the patient would like to be (visually with pictures). Then I do the math to add or subtract from the implant size of the patient in the picture if the patient in the picture PRE OP is different from the patient in the consult (and it usually is).    I rarely miss on this step in the thousands of augmentation patients that I have treated over the years.

We decided on a breast lift with High Profile ( I really like this implant shape) silicone implants (550 cc's) in the submuscular position.   The choice of the final size is strictly determined by the patient's wishes as the ability to put in an implant of any size can be easily done on just about any patient. ( You might read that a doctor would only use an implant of up to 350 cc's or so.  Why?  This would be an error in this patient when you see what she is trying to achieve.)   With the breast lift and augmentation,  we will perform a tummy tuck and liposuction of any fatty tissue in the axilla/upper back/waist and hips.  If it is there, it is going to be in a container pretty soon !  This patient has good muscular tone and you can see this in her pre op front view.

It is important for the patient to have the final input on all of the decisions but it is equally important for the doctor to give straight forward answers to questions and give suggestions as to other ideas that the patient might think interesting..  A give and take during the consult is very creative and this allows for the best approach.

This patient is seen at 4 months.  This is very early in the shaping process as there will be changes continuing for up to a year.  You are seeing pink scars and they will fade with the time mentioned.
